NRHA Quality Care Award Winner Gold Seal of Approval

Quality of patient care has always been a priority for our staff at Memorial Health Center. Through ongoing process improvement projects, continued accreditation from nationally recognized healthcare quality experts, and with patient satisfaction survey programs, our healthcare organization has made a commitment to continually improve the quality of the care provided to our patients by our staff and providers. Our intent is to keep our patients and the communities we serve well informed about the quality of our services.

Quality Reports:

Quality - a definition.

Quality at Memorial Health Center is defined as finding better ways to provide services and care for patients. We utilize Continuous Quality Improvement (CQI) methods to assist in identifying opportunities for performance improvement and in developing solutions to problems. Quality related activities usually involve a measurement of performance. We measure first as a basis for making judgments and decisions, and second, as a basis for future improvements.

Quality and Performance Improvement are Monitored Everyday.

Our organization's commitment to quality and performance improvement is demonstrated through the membership of our Quality Leadership Committee. This committee serves to determine and support the education and training needs of Memorial Health Center related to quality and performance improvement. Additionally, this committee prioritizes quality initiatives and other performance improvement projects based on actual or potential impact on patient care and services. Emphasis is place on improving patient outcomes.

Staff Involvement in Quality Improvement is an Expectation.

Staff at Memorial Health Center is provided an introduction to quality improvement during their employee orientation. Staff understanding of and involvement with quality improvement is necessary for these efforts to be successful for the long term. Our commitment to quality includes a regular survey of our employees to identify areas for leadership improvement.

Healthcare Quality Experts - Our Accreditation.

Memorial Health Center is accredited by the Joint Commission.

The Joint Commission has been overseeing the quality of health care for half a century, continually adapting their evaluation process to fit the way healthcare is delivered today. As an independent organization, they provide regular surveys to look at quality and performance improvement information, including sending experts to our hospital to interview our employees, doctors and volunteers. They will even speak directly with our patients and their family members.

During our most recent survey in 2006, Memorial Health Center received a three-year accreditation.

For our patients and the communities we serve, the accreditation should represent a "quality seal of approval". The Joint Commission requires that the organization meet rigorous standards. Our staff and providers spend a significant amount of time preparing for planned survey visits and must maintain a state of constant preparedness for unannounced surveys.

In 2008, our Memorial Nursing & Rehab Center was pleased to announce that its unannounced state inspection was deficiency-free! Fewer than 5% of the skilled nursing home facilities in the state of Wisconsin achieve this milestone. We're proud of our medical professionals, their compassion, and passion for excellence. This most certainly is a validation of the quality of care we provide your loved ones.

Memorial Health Center Clinics' laboratories are accredited by C.O.L.A. It is very common for our highly technical and precision-based labs to achieve perfect evaluation scores.
