Memorial Health Center's Senior Care Services Include
Cedar Court Apartments – an independent low-income housing complex that accepts seniors or those who have a disability diagnosis. The major benefit of living at Cedar Court is its proximity to Memorial Health Center; it’s within walking distance to physicians, a pharmacy, gift shop, and Memorial Health Center’s Med-Rock Café. Plus, there are no staircases in the facility; everything is on a single level.

Country Gardens Assisted Living Complex – a RCAC, Residential Care Apartment Complex, it can provide medication assistance and has universal caregivers onsite 24 hours - 7days a week. Housekeeping, laundry, bathing assistance, home-cooked meals 3-times a day, and exercise and social programs are just some of the amenities. Rent is all-inclusive; there are no hidden costs.

Cedar Lane Assisted Living Apartments – CBRF resident care. Ten (10) spacious private apartments are attached to Memorial Nursing & Rehab Center (Medford). This assisted living option provides supervised living where individuals can enjoy life to its fullest while receiving assistance with activities of daily living.

Memorial Nursing & Rehab Center – is a 101-bed skilled nursing facility that is literally connected to Memorial Health Center Hospital. Multiple care options, in a relaxed and home-like environment are available.

- General and Restorative Nursing Services
These services are designed for those who no longer can be cared for at home or in an assisted living setting due to illness, injury, or other physical limitations. - Skilled Nursing Services
Benefit those who may have suffered from a prolonged acute or chronic illness or may have had a recent illness, injury or surgical procedure. These individuals are medically stable, but may require an increased frequency of nursing assessment and monitoring. - Skilled Rehabilitation Services - Bridge to Home
We will provide your loved one with the therapeutic services they need in a homelike atmosphere. Our skilled nurses, licensed therapists (physical, occupational, speech, respiratory), and trusted physicians are available 24 hours a day. We will work with your loved one daily, until they are at the optimal level of independence to return home safely. Most individuals in the rehabilitative center are recovering from a fall, stroke or surgery.
In 2008, 2009 & 2010, Memorial Nursing & Rehab Center, based on the quality of care it provided, was ranked among the nation's Top 12%. Zero deficiencies (as identified by state inspectors) can only be claimed by 5% or less of the nursing home facilities in the United States. Memorial Nursing & Rehab Center was one of them too!
Memorial Nursing & Rehab Center offers services and an individualized plan of care to meet the needs of those facing Alzheimer’s Disease, Dementia, or other memory challenges.
In cooperation with Hope Hospice and Palliative Care Inc., Memorial Nursing & Rehab Center provides a holistic commitment to one’s end of life.
For all Memorial Nursing & Rehab Center residents, certified activity directors, and licensed physical, occupational, and speech therapists provide a wide range of daily therapeutic and active leisure activities including arts and crafts, reading groups, brain games to stimulate memory and other cognitive functions, exercise, music, church services, and/or transportation and assistance to enjoy community festivals, concerts, and fishing in our local lakes. Our knowledgeable and experienced staff provides sensory stimulation and one-to-one interaction to individualize each resident’s needs.
